Flute workshops with Dr Jessica Quiñones - BOOKING FORM

    To book for the flute workshop on Saturday 10th or Sunday 11th October, please complete and submit the form below. 
    The fee of £60 can be paid by Paypal, bank transfer or cheque - please select your preferred option below. When we receive your form, we'll send you details of how to pay, along with a brief anonymous questionnaire to complete so we can get the best idea of what everyone wants from the day. A place on the workshop will be allocated once payment is received (payments are non-refundable). If you are available on both days, please state your preferred day on the form, and we will try to allocate this where possible.
